Early drug-induced hepatotoxicity in newly diagnosed HIV-positive patients on ARV therapy: A retrospective follow-up study of liver function profiles

Antiretroviral therapy (ART) is a primary therapeutic modality for managing individuals with Human Immunodeficiency Virus (HIV) infection, and its efficacy has been established. However, the safety profiles of ART need to be continually monitored, including early elevated liver function test (LFT) after antiretroviral (ARV) initiation. This study aimed assess incidence abnormal LFT among HIV-positive patients receiving initial ARV identify factors associated LFT. A retrospective cohort without control group summarised medical records from Dr Sardjito General Hospital, Yogyakarta between January 2014 December 2021. The subjects were adult HIV taking their first underwent follow-up visits at least two weeks. Study outcomes levels, LFT, during 2 wk, 6 mo, 12 mo. Univariate multivariate analyses will performed significance level p<0.05. total 137 203 retrieved records. subjects' mean age was 33.4 years, predominantly male, younger, unmarried, in stage comorbidities. findings showed significant increase alanine transaminase (ALT) all three visits: wk (p=0.02), mo (p=0.003), (p=0.001) an aspartate aminotransferase (AST) (p=0.007) (p=0.04). Abnormal levels (AST and/or ALT) observed 23.4% normal baseline, ALT being more common. Furthermore, homosexuality identified as independent factor (AOR=3.1; 95% CI 1.27-7.51; p=0.01). indicate exceptionally occurrence LFTs initiating ARVs. significantly identifying homosexual, where hepatitis co-infection may contributing factor. limited design measured variables warrant further investigation.
